Guidelines

1. This form consists of three (3) parts:
    a. State and name of Local Authority;
    b. Details of Respondent; and
    c. Feedback on facilities dan service provided by Local Authority.

2. Respondents are required to answer all 24 questions. It will take 2-3 minutes to complete this form.

3. Feedbacks will solely be used for the purpose of improving service delivery at Local Authorities.
